For purposes of the following authorization statement, “I” and “my” mean the
person who is the checking or savings account holder.
I hereby authorize Doc2Doc Lending, its agents, successors and/or assigns (“Lender”) to
credit my designated checking or savings account by disbursing funds to said account in
accordance with the terms of my Loan Agreement with Lender. I acknowledge that the
origination of ACH transactions to my account must comply with NACHA regulations,
OFAC and U.S. Law.
I understand that this authorization also allows for ACH reversals to be originated in order
to correct errors.
I understand that I am responsible for providing a valid checking or savings account
number and my corresponding bank’s ACH routing number, and that the Lender shall not
be liable for any errors that may result from my providing incorrect account information.
I may terminate this authorization by providing Lender with written notice. This authority
is to remain in full force and effect until the Lender has received such written notification
from me and has had reasonable opportunity to act on such notification.
